Title: Analog front end design

Hello everyone,
Being more of a digital designer, I am currently stuck at designing an analog
front end for my DSP system. This maybe a fairly basic question for most of you
analog experts, so I really hope to get some helpful answers to this.

The requirements for my analog signal conditioning circuit are as follows:
I/P: Differential +/-2.5V
O/P: Single ended 0-5V
Power supplies: Either dual +/-5V or single 5V (would prefer single supply, but if solution not possible with single, then dual supply is fine).
Unity gain bandwidth: Atleast 20MHz
Slew rate: Atleast 10V/us.
No.of channels: 4 in one IC.

Moreover, this being a fixed voltage conversion, I would really like to have the biasing and/or gain setting resistors for the op-amp to be internally available. Having to connect the resistors externally is something I would like to consider only if no solutions exist with in-built components.
I could not get an op-amp that matches the above requirements by going through the TI/Analog Devices op-amp selection guide, but then I could have missed out on something you guys might be aware of.

I really appreciate help with this.
