If you need to derive the gain of an differential output in db...
What you need to calculate is  

db(out-in) or
db(out)-db(in)

dbm(out) - dbm(in) = dB gain

or

dbV(out) - dbV(in) = dB gain

If you want to be fussy about definition, dB is a ratio, so there is no such thing as "dB" of a singular measurement

Just to nitpick!!

db(out-in) is not correct.
Correct would be db(out/in). This is equivalent to db(out)-db(in).
