What's the meaning for ACM=12 at Hspice Level=49?
And What's the difference between ACM=12 and ACM=0,1,2,3?

I believe this is covered in the hspice model documentation - it's something to do with which diode equations are used - Berkeley or HSPICE native models.

I suggest you read that part of the HSPICE manual.
