dependent sources in spectre

Does anyone know how to covert the following hspice code to spectre in cadence environment?

F21 vgate vs POLY(2) vsens4 vsens5 0 0.3638 -0.3638

I see a CCCS in analogLib, but i'm not sure how to use it for the above statement.
